What the achievement asks
Read all six Notes On The Machine Speakers in The Ringing Deeps. Five points, account-wide.
The coordinates are corrected for patch 11.1
Patch 11.1 added Undermine and stretched The Ringing Deeps map sideways. Every coordinate list written before that patch is roughly four points too far east, which is why people report standing in solid rock.
The numbers below have that shift applied. The correction is close to four points rather than exactly four, so if a coordinate looks empty, sweep a short distance east and west of it rather than giving up.
The six below are the corrected values published by a player who re-measured them after the patch.

Fragment IV is the awkward one
Fragment IV is not on the ground. It sits on a wooden frame, second from the very top, and reaching it means hovering precisely rather than landing.
Switch to Steady Flight for this one. Skyriding is fast and it is bad at holding a position next to a narrow beam, and players who fought it in Skyriding gave up and switched. Turn it back on afterwards.
Where it fits
One of the pieces of You Xal Not Pass, which rewards the Shadow of Doubt mount and is part of Worldsoul-Searching, the War Within meta. Collect the plaques of Rocked to Sleep and the consoles of Not So Quick Fix on the same lap: all three are pure travel through the same zone.
