What the achievement asks
Complete the six storylines of the Emerald Dream campaign. Ten points, account-wide.

They run in order and each one opens the next, so there is nothing to plan: take the campaign quest and keep going.
The gate is in the previous patch
The Emerald Dream campaign does not start until the Zaralek Cavern campaign is finished. The quest that carries you across is Tracking Down Incarnates, and it will not offer until Zaralek is complete.
Players who skipped ahead have found themselves having to go back and work through the whole of Zaralek before the Dream would open at all. If Enter the Dream is not available to you, that is where to look, not at the Dream itself. The Zaralek campaign is Embers of Neltharion in the same meta, so it is work you owe regardless.
The chapters were released a week apart
When the patch was current, the campaign unlocked in stages: four chapters in the first week, two more in the second, and the last one in the third. That gate is long gone and every chapter is available now, but it explains the older comments from people who did everything available and stopped at four of six.
The final chapter follows the Amirdrassil story. At release, killing Fyrakk in Raid Finder did not advance it, which caught out a number of people who assumed the raid difficulty was interchangeable. If your last chapter refuses to move, check the chapter before it is genuinely closed before concluding anything: an unfinished earlier storyline is by far the more common cause.
Where it fits
One of six under Dragon Quests, a piece of A World Awoken. Doing it also opens the Emerald Dream's own branch, so it is worth finishing before you start collecting anything in that zone.
