What the achievement asks
A Flawless run on layer 16 in Coldheart Interstitia. Ten points.
Coldheart Interstitia has one quirk in its favour. Its escalating debuff pushes enemy health up rather than enemy damage, so a slow run here gets longer rather than more lethal. For a scored run that is a good trade, since the thing that actually ends attempts is dying.
Flawless is a score
It does not mean an unblemished run. Torghast rates every attempt out of a points total, and Flawless is the top band, which players put at 200 points. Mistakes are affordable. Even a death is survivable: it costs 20 points, not the run.
The bonuses that carry a modern run are speed and completion, and they arrive on their own when everything dies in one hit. The ones worth steering into deliberately cost nothing:

Each death is a 20-point fine and it wakes the Tarragrue, who then races you up the tower. The points you can absorb. The Tarragrue is the reason to be careful.
Two things to do before you walk in
Buy The Adamant Vaults perk, 100 Tower Knowledge in the Box of Many Things. Any Flawless run at layer 9 or higher turns the floor 5 exit portal red and adds two more floors, and stepping through it is all of Adamant Vaults. This run qualifies. If you have not bought the perk, it does not.
Check the daily roll. Blessings and Torments are fixed per wing and reset with the daily quests. A bad pairing here today is a good pairing here tomorrow, and switching wings costs nothing.
Layer 16 pays for layer 12
Clearing this at 16 also awards the layer 12 version for the same wing if you do not already hold it. Running 12 first is wasted effort.
